BACKGROUND AND OBJECTIVES: Bone remodeling is characterized as a cyclic and lengthy process. It is currently accepted that not only this dynamics is triggered by a biological process, but also biochemical, electrical, and mechanical stimuli are key factors for the maintenance of bone tissue. The hypothesis that  low-level laser therapy  ( LLLT ) may favor bone repair has been suggested. The purpose of this study was to evaluate the bone repair in defects created in rat lower jaws after stimulation with infrared  LLLT  directly on the injured tissue.

Summary Low-level laser therapy (LLLT) has been proposed for a wide variety of uses, including wound healing, tuberculosis, and musculoskeletal conditions such as osteoarthritis, rheumatoid arthritis, fibromyalgia and carpal tunnel syndrome. There is insufficient evidence in the published, peer-reviewed scientific literature to demonstrate that LLLT is effective for these conditions or other medical conditions. Large, well-designed clinical trials are needed to demonstrate the effectiveness of LLLT for the proposed conditions.

Abstract Objective: The objective of this review was to systematically identify experimental studies of non-ablative laser irradiation (LI) on peripheral nerve morphology, physiology, and function. The findings were then evaluated with special reference to the neurophysiology of pain and implications for the analgesic effects of low-level laser therapy (LLLT).

In summary, the available literature has not shown laser therapy to be as effective as established treatments for the treatment of various musculoskeletal conditions, including carpal tunnel syndrome, or for wound healing. Outcomes of studies comparing LLLT with sham laser therapy for treatment of pain are inconsistent and generally involve small numbers of participants. There is some evidence of a dose-dependent effect in the treatment of tendinopathies. Randomized controlled trials involving larger numbers of patients comparing low-lever laser therapy with established treatments and reporting complete dosage information are required. Evidence for LLLT for many conditions including chronic wounds and nerve damage is extremely limited and FDA clearance has not been given for use of LLLT for these indications. Therefore, low-level laser therapy is considered investigational.

RESULTS: Twelve studies met all selection criteria for inclusion in the review: 4 studies addressed the use of therapeutic exercise interventions, 2 studies examined the use of acupuncture, and 6 studies examined electrophysical modalities. Two studies provided evidence in support of postural exercises to reduce pain and to improve function and oral opening. One study provided evidence for the use of manual therapy in combination with active exercises to reduce pain and to improve oral opening. One study provided evidence in support of acupuncture to reduce pain when compared with no treatment; however, in another study no significant differences in pain outcomes were found between acupuncture and sham acupuncture. Significant improvements in oral opening were found with muscular awareness relaxation therapy, biofeedback training, and  low-level laser therapy  treatment.

METHODS: A total of 52 recreational athletes with chronic Achilles tendinopathy symptoms were randomized to groups receiving either EE +  LLLT  or EE + placebo  LLLT  over 8 weeks in a blinded manner. Low-level laser therapy (lambda = 820 nm) was administered in 12 sessions by irradiating 6 points along the Achilles tendon with a power density of 60 mW/cm(2) and a total dose of 5.4 J per session.
